A living will, which is a type of advance directive, is a written document that expresses your personal values, beliefs, and wishes related to medical treatment and dying.

  • The name of a nominated person who knows your wishes and intentions, and who will assist in making critical medical decisions if you cannot do so.
  • If desired, refusal of a particular treatment or procedure, such as tube feeding, intravenous infusions, blood transfusions, or antibiotic therapy.
  • Specified circumstances in which no life-sustaining treatment, such as resuscitation or mechanical ventilation, should be given.
  • Your wishes regarding organ donation after your death.
